New Patient Registration

The procedure for registering new patients is as follows: 

If the list is open, you can register online or the receptionist will give you the Registration Form to complete.  

We will require a completed form for every member of your household who wishes to register.

You will need to attend an appointment with the Practice Nurse. 

When the patient has been seen by the Practice Nurse and if they still wish to be registered, the information will be put onto the computer and records requested from the PCSE.

The GP accepting new patients is Dr Valasapalli.

If the practice list is closed the patient will be given a letter of explanation and instructions on who to contact in order for find a GP.

The application may take a few days to complete. If you have not heard back from the surgery within 5 working days, please contact the surgery by phoning 0300 790 0169   or email: surgeryinbox@stoke.nhs.uk

Named Accountable GP

All patients registered at a GP practice are to be allocated a named accountable GP.

What does ‘accountable’ mean?

The GP contract requires the named accountable GP to be responsible for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of those patients where required, on the patients behalf.

However, this does not mean that they will be the only GP or clinician who will provide care to that patient. This GP may be a partner or a salaried GP.

Can patients choose their own named GP?

In the first instance, patients will simply be allocated a named GP. However, if a patient requests a particular GP, reasonable effort will be made to accommodate their preference.

Do patients have to see their named GP when they book an appointment?

No. Patients can and should feel free to choose to see any GP or nurse in the practice in line with current arrangements. However, if their preferred choice of GP is not available, an alternative will be offered.

As of January 2016 all patents registered with Dr Miles or Dr Valasapalli have been allocated a named GP.
