If you change your name, address or telephone number please give full details of the change at reception as soon as possible. Please be aware we can only make a change of either of the above if you provide us with proof of address/ or name change. If you move away from the practice area you will be asked to find a doctor in your new area.

Please also let us know if your telephone number or email address changes.

You can also submit a Change of Contact Details form online but you will need to bring in proof of name or address changes with you on your next visit to the practice.
